Epidural: how it works and what to expect

The most commonly used pain relief method during labor in North America. Here's how it's given, its effects, and its limits.

An epidural is an injection of local anesthetic into the space around the spinal cord, in the lower back, that blocks the pain of contractions while generally preserving some sensation and the ability to move your legs, depending on the dose used.

It's usually placed by an anesthesiologist once labor is well established, with you sitting or lying on your side. A brief discomfort at the injection site is normal, but pain relief typically kicks in within 10 to 20 minutes and can be adjusted throughout labor.

Possible side effects include a drop in blood pressure, itching, mild fever, or, less commonly, a post-puncture headache. Medical staff closely monitor both mother and baby once the epidural is in place.

An epidural doesn't remove the sensation of pressure while pushing, and it can, in some cases, slightly lengthen that stage. Choosing to have one — or not — is entirely personal: there's no single 'right' way to give birth, only the one that fits your situation and preferences.
