Giving birth to twins: what's different and what's challenging

Vaginal, cesarean, or both: how a twin birth unfolds and what shapes the options available.

A twin pregnancy is monitored more closely than a singleton one, with more frequent ultrasounds to track both babies' growth, and birth tends to happen a bit earlier, generally between 36 and 38 weeks depending on the type of twins and how the pregnancy is progressing.

Whether a vaginal birth is possible mostly depends on the babies' positions: if the first twin (closest to the exit) is head-down, a vaginal birth often remains an option even if the second twin is positioned differently, since the team can sometimes help reposition them after the first baby is born.

A cesarean is generally recommended if the first twin isn't head-down, or in certain situations such as twins sharing the same amniotic sac. It can also happen, though rarely, that a birth starts vaginally for the first baby and ends in a cesarean for the second.

Whichever mode of birth is chosen, a delivery room equipped for a multiple birth, with extra staff ready to care for each baby separately, is generally arranged in advance — allowing for quick, safe care right from birth.
