Post-term pregnancy and induction: what happens after 41 weeks?

Once past the due date, closer monitoring kicks in — here's how it works and why induction is often offered.

A pregnancy is called 'post-term' after 42 completed weeks, and 'late term' between 41 and 42 weeks. Only about 5 to 10% of pregnancies actually go past 42 weeks, with most babies born before that point.

After 41 weeks, the placenta can start functioning a bit less well for some women, which slightly raises the risk of complications such as reduced amniotic fluid or lower oxygen delivery to the baby. That's why more frequent monitoring — fetal wellbeing tests, checking amniotic fluid — is generally put in place from that point on.

Many guidelines now recommend offering induction between 41 and 42 weeks rather than waiting for spontaneous labor, since the evidence shows this approach reduces the risk of certain complications without increasing the cesarean rate, contrary to what was once believed.

The final decision stays a discussion between you and your care team, who'll take into account the state of your cervix, the baby's wellbeing, and your own preferences. Continuing to track the baby's movements and reporting any decrease remains important during this waiting period.
